PHIA & Confidentiality

Personal Health Information Act (PHIA)

Aurevita Health Group operates in compliance with Manitoba's Personal Health Information Act (PHIA), which governs the collection, use, disclosure, and protection of personal health information. PHIA gives individuals the right to access their health records and requires health care trustees to safeguard that information.

Confidentiality Statement

All Aurevita staff, contractors, and volunteers are required to sign a Declaration of Confidentiality upon hire. They receive training on the secure handling of personal health information and are bound by law to maintain confidentiality. Breach of confidentiality is subject to disciplinary action and may result in legal consequences under PHIA.

Your Rights Under PHIA

You have the right to:

Every client of Aurevita Health Group has the right to:

  • Access their own records and request corrections

Client Consent Process

Informed consent is obtained before the start of any service. Our process includes:

  • A comprehensive assessment of your care needs
  • A clear explanation of the services to be provided, including scope, schedule, and fees
  • Review of privacy practices and how your information will be handled
  • Written consent signed by the client or authorized representative
  • Ongoing communication to ensure consent remains informed and current

Clients may withdraw or modify their consent at any time by contacting our office.

Complaints Process

We are committed to providing the highest quality of care. If you have a concern, we encourage you to share it with us so we can address it promptly and fairly.

How to File a Complaint

Step 1: Contact Your Care Team

Speak directly with your caregiver or their supervisor. Many concerns can be resolved quickly at this level.

Step 2: Contact Management

If your concern is not resolved, please contact our office by phone or email. We will acknowledge your complaint within 2 business days.

Step 3: Formal Complaint

If further action is needed, you may submit a formal written complaint to our Privacy & Access Officer. We will investigate and respond in writing within 30 days.

Step 4: External Review

If you are not satisfied with our response, you have the right to file a complaint with the Manitoba Ombudsman at ombudsman.mb.ca or call 1-800-665-0531.

Abuse Prevention & Safeguarding

Aurevita Health Group is committed to protecting the dignity, safety, and well-being of every client. We have zero tolerance for abuse, neglect, or exploitation in any form.

Our Safeguarding Measures

  • All staff undergo criminal record checks, adult abuse registry checks, and child abuse registry checks prior to employment
  • Reference verification and employment history review for every hire
  • Mandatory training on abuse prevention, recognition, and reporting
  • Standard First Aid and CPR certification for all direct care staff
  • Clear policies for reporting suspected abuse or neglect, with protection for whistleblowers
  • Regular supervisory visits and client wellness checks
  • Ongoing staff training in Workplace Health and Safety

Recognizing Abuse

Abuse can be physical, emotional, financial, sexual, or related to neglect. We encourage clients, families, and staff to report any concerns immediately.

Insurance & Bonding

Aurevita Health Group carries comprehensive commercial general liability insurance and professional liability insurance. All staff are covered under our insurance policies. Proof of insurance is available upon request.
